Health Permit Reissuance Fee 15% of past-due amount
Health Re-Inspection (closures due to critical violations)
Closed child care and food establishments $125
Closed swimming pools/spas and hotel/motel $125
Pre-Permit Inspection Request $125
Food Manager Certificate $15 per card
Literature Sales $.10 per page
Pool Operator's Course $65 per person
Health Card Fees
In-House Training (Food and Daycare) $16

Health Card Reciprocal Fees

Mobile Food Vendor $65

Farmer's Market $65 - $200 based on number of produce vendors
Change of Ownership Inspection Fee $125
Misc Revenue $5 per duplicate permit or trainee card
Temporary Health Permit
Private Event, i.e., food shows not for general public $200
Food and Beverage Booth open to general public $50 per day + $15 each additional day
Administrative Fee, if temporary permit is not
purchased 3 days prior to event
$20 per vendor request
